NHS England (NHSE) are running a national pilot of ‘community connectors’ as part of the Health Inequalities programme. They are looking for a Voluntary, Community and Social Enterprise Sector (VCSE) sector partner to work with the GM Health and Social Care Partnership to put in a bid for Wave 2 (national deadline is 28th April).
NHS England are looking for a VCSE partner that is well placed to lead work to test their model of community connectors in relation to one or more of these communities:
• People living with a disability
• Families at the time of pregnancy/birth
The national funding is part of CORE20PLUS5 programme which is focusing on recruiting and training ‘community connectors’ to capture insight and experiences and to work alongside health and care partners to improve access to prevention and screening services for these 5 clinical priorities:
• Cancer screening
• COPD screening
• CVD screening
• Health checks for people with Serious Mental Illness
• Maternal health
Funding for 22/23 is £72k with potential for further funding of £20k in year 2 and 3.
